- Support for Medical Heart Devices (ECMO, ABIOMED, THORATEC, HEARTMATE) with a subsidy of up to $20,000
- Patients who are unable to settle the outstanding balance of their heart transplant expenses and subsequent follow-up treatment fees can request additional financial support from SHF (maximum of $20,000).
- Offers financial aid for patients who suffer income loss or need to take time off work for cardiac rehabilitation or routine medical appointments.
- Provides CabCharge cards to assist with the cost of frequent follow-up hospital visits for heart patients.
- Sponsorship of medical devices (eg: BP monitors and heart rate monitors) and mobility devices (eg: wheelchairs)

- Heart patients may approach their hospital medical social workers for help to facilitate the application.
