REACH Community Mental Health Team
REACH Community Services’ community mental health team provides free, confidential support, outreach, psychosocial interventions, and case management for adults in Bukit Gombak and Hong Kah North, plus a caregiver support group for dementia caregivers.
Overview
A community-based mental health service run by REACH Community Services that acts as a first-stop, in-person support for residents in Bukit Gombak and Hong Kah North (aged 18 and above).
- Early identification of signs and symptoms of mental health conditions and dementia
- Targeted outreach and education to improve mental health and dementia literacy for residents and caregivers
- Psychosocial interventions and techniques to reduce escalation of mental health needs
- Case management to coordinate care with health and community partners
- Collaboration with stakeholders to support persons with mental health conditions and dementia in the community
- Caregiver Support Group for caregivers of persons with dementia (English sessions, Friday afternoons once every two months; complementary elder-sitting may be arranged)
- Confidential support with no out-of-pocket cost

Residents aged 18 and above living in Bukit Gombak and Hong Kah North. Caregiver Support Group is for caregivers of persons with dementia (sessions conducted in English). No out-of-pocket cost.
