Mental Wellness Outreach for Seniors (CREST Programme)
TOUCH’s CREST programme in Cheng San–Seletar provides community-based mental wellness outreach, screening, emotional support, caregiver resources, and referrals for seniors at risk of or living with dementia and other mental health conditions.
Overview
The Community Resource, Engagement & Support Team (CREST) by TOUCH promotes early detection of dementia and mental health conditions for residents in Cheng San–Seletar.
Key features and support include:
- Community outreach and education to raise awareness of dementia and mental health conditions among seniors
- Mental health screening to identify at‑risk individuals early
- Emotional support and a listening ear for persons with or at risk of dementia and mental health issues
- Guidance and resources for caregivers to navigate support options
- Preventive, senior-friendly activities (e.g., walks and SilverPad sessions) to support well-being and stimulate cognitive function
- Linkage and referrals to relevant health and social care services closer to home
Developed under the national Community Mental Health Masterplan, the service supports both seniors and their caregivers along the mental wellness journey within their community.

- Aged 18 and above
- Resides in the Cheng San–Seletar constituency
- Diagnosed with dementia, depression, or other mental health issues OR identified to be at risk of developing these conditions
