Overview
CREST HNF @ Sengkang is a community mental health outreach service by Home Nursing Foundation that supports residents in Fernvale and Jalan Kayu aged 18 and above.
- Raises awareness of mental health conditions and dementia through community outreach, talks and workshops
- Promotes early recognition of at-risk individuals and facilitates mood and memory screenings
- Provides basic emotional support and ongoing casework follow-up, including home visits
- Links individuals and caregivers to appropriate health and social care services
- Collaborates with community partners to coordinate mental health and dementia education in the region
- Promotes Dementia Friendly Initiatives to build an inclusive, supportive environment for persons with dementia and their caregivers
