Homecare provides medical, nursing, personal care and rehabilitation services for people who need care at home. Services are available in parts of Singapore’s North East and North West regions.
Overview
Home-based intermediate and long-term care services include:
- Home medical care: chronic disease and medication management.
- Home nursing: clinical care such as tube, wound, stoma and tracheostomy care, medication packing and administration, and chronic disease monitoring.
- Enhanced Home Personal Care (HPC+): housekeeping, personal care, custodial support, and fall and movement monitoring and response.
- Home therapy: physical rehabilitation and exercise training to improve function and reduce further deterioration.
- Nursing services also include health education and caregiver training and support.

Eligibility details
Services are for homebound clients. Home Medical is for seniors with chronic conditions or disabilities who need continuing or long-term medical care. Home Nursing is for clients with chronic conditions or disabilities who need nursing care or procedures provided by a trained nurse. Home Therapy is for clients who need home rehabilitation. HPC+ is for clients assessed as needing help with activities of daily living (ADL) and/or instrumental activities of daily living (IADL); eligibility is subject to the Approved Provider’s assessment using the InterRAI assessment tool.
