Fei Yue Community Services
Advance Care Planning (ACP)
Fei Yue offers facilitated Advance Care Planning discussions and access through a partner clinic to LPA and AMD services. ACP is free, while LPA and AMD have a stated net service fee of $50.
Overview
Advance Care Planning (ACP) helps people express their values and preferences for future medical and personal care through guided discussions. It can cover choices in serious illness, preferred care and end-of-life arrangements, and appointing a Nominated Health Spokesperson to communicate wishes if the person cannot do so.
The service also connects clients with a partner clinic for Lasting Power of Attorney (LPA) and Advance Medical Directive (AMD) services. LPA lets a trusted donee make decisions if the donor loses mental capacity; AMD records a person’s choice not to receive extraordinary life-prolonging treatment if they become terminally ill. ACP is free; the partner clinic charges a net service fee of $50 for LPA and AMD.
