Eligibility details
- Singapore Citizen or Permanent Resident
- Have a confirmed cancer diagnosis
- Require regular hospital visits for cancer treatment
- Meet SCS’s financial assessment criteria
How to apply
- Prepare the following documents required:
- Copy of NRIC (front and back) of patient
- Copy of Medifund, Blue/Orange CHAS, MFAC or PA card of patient
- Copy of upcoming medical appointments schedule
- Completed application form and endorsement by doctor, nurse or medical social worker
- Copy of medical report(s) for Ambulance Service applications
- Approach your doctor, nurse or medical social worker to complete and submit your SCS Transport application form together with the required supporting documents.
