Community-based mental health and dementia support by AIC, offering outreach, psychotherapy, caregiver support, and coordinated care with GPs, polyclinics, and hospitals. Includes youth outreach, post-diagnostic dementia support, aftercare, and primary care integration.
Overview
CREST is a community safety network supporting individuals with or at risk of depression, dementia, and other mental health conditions, as well as their caregivers.
We provide resources and assistance to help them continue care at home and in the community.
Community Outreach Teams, operated by social service agencies, are available in neighbourhoods for support.

What you receive
Eligibility details
Open to individuals in the community with mental health issues or dementia, and their caregivers. Youth experiencing psychological distress can receive support via CREST-Youth. Newly diagnosed persons living with dementia and their caregivers may receive post-diagnostic support (for six months from referral). Urgent/severe cases surfaced by GP partners and COMIT may be triaged by ASCAT psychiatrists for fast-track hospital services. Specific clinical suitability and referrals may be required depending on the programme.
