Overview
AWWA’s Community Resource, Engagement and Support Team (CREST) is a community-based, first-stop touchpoint for mental health and dementia needs in the Ang Mo Kio region. It aims to identify at-risk individuals early and provide timely psychosocial support for them and their caregivers.
Key supports and activities include:
- Psychoeducational events, talks and workshops on mental health conditions and dementia
- Individualised casework and ongoing monitoring through home visits
- Mood and memory screening to flag early concerns
- Information provision and referrals to appropriate services and partners
- Psychosocial interventions and practical support to help individuals and caregivers manage day-to-day challenges
- Collaboration with community partners to strengthen local support networks
