Rise to Wellness - Integrated Wellness Recovery Programme

RISE to Wellness Integrated Recovery Programme is an integrated, structured wellness recovery programme that takes peers, i.e. persons who are able to engage in social settings and communicate their wellness needs but have yet to achieve full recovery, on the journey from struggling to coping or thriving. Every peer will be supported by a Care Manager who will seek to understand the specific needs of the peer and curate a set of activities, among those currently offered by PSALT Care, to help peers achieve their recovery goals. Peers on this programme would be expected to demonstrate some level of personal responsibility and interest in participating in the activities recommended by their Care Manager.
