Eligibility details
Eligibility varies by service. For MOH subsidies in nursing homes, patients must meet means test criteria and be referred by AIC. Home-based services are for frail and home-bound elderly who find it difficult to access community services. Meals-on-wheels is for home-bound elderly unable to buy or prepare meals and without caregivers. Medical escort and transport is for seniors unable to attend appointments alone and without suitable caregivers. Rehabilitation and sheltered homes are for persons with stabilised mental health conditions in recovery. Inpatient hospice care is for end-of-life patients who cannot be cared for at home.
How to apply
Application processes differ by service. For meals-on-wheels, apply directly for the service. For MOH-subsidised nursing home care, patients must be referred by AIC. For other services, contact the relevant service provider or eldercare centre. Some VWOs provide financial and social assistance for those unable to afford fees.
